Currently (since the May 9, 2013 patch): randomly dropped from Glory, Cosmic, or Prestige packs. As of the May 16, 2013 patch, you will always get one item (out of the five possible) from each pack.
From the Tempus rollout (October 18, 2012) to the May 9, 2013 patch: unavailable, except through old packs from the Time Traveling Merchant.
Before the Tempus rollout: randomly dropped from Glory, Cosmic, or Prestige packs, in any of six different colors. The colored items were converted automatically to the new items with the May 9, 2013 patch. See here for full details.
